Agreement to Terms
of Telephone Consultations

All Patients Must Sign Agreement Before Consultation. As of October 13, 2003, the federal government imposed HIPAA regulations on all health care practitioners in the USA. Under the influence of these regulations, we must have patients formally agree to the conditions for consultations with Dr. Lowe. Thanks for your cooperation with this requirement.

Before your first consultation, please provide us with the signed copy of the "Agreement Form." The signed copy of the Form verifies that you understand and agree to the terms of telephone consultations with Dr. Lowe.
